I would like to make reservations for "That's Amore!" on February 12th, 2011, from 6:30-8:00 pm at Westlake UMC. I understand that, if I want to have childcare provided for my children, I must make reservations for childcare before February 8th in order that Westlake UMC will have time to arrange for the right number of childcare staff. The advance ticket price per child will be $10, including pizza and childcare.

I want to make reservations for the dinner and musical revue, "That's Amore!" on February 12th, at 6:00-8:00 p.m. I understand that the price of $10 for a child's ticket includes pizza dinner and babysitting.
These reservations are "will-call" reservations and need to be paid by February 8th or the reservations will be cancelled. Payment can be made in the church office, in the Foyer of the Worship Center on Sundays, or mailed to That's Amore, Westlake UMC, 1460 Red Bud Trail, Austin, TX, 78746 (received by February 8th).
